CareLink exists because of a pattern every clinician in Far North Queensland knows: good hospital care, good GPs, and a gap between them where families are left holding clinical work they were never trained for.

Why we exist

The discharge worked. The week after didn’t.

Far North Queensland has excellent hospitals and committed GPs. What it has lacked is a dedicated clinical layer at home: senior nurses who can take a discharge plan, a DVA referral or a worried family’s phone call and turn it into safe, documented care in the place people actually recover. That is the whole of what CareLink does. We are a clinical nursing service. We don’t do cleaning, transport or companionship, and we won’t pretend to. We do the nursing, and we do it properly.

How we work
Senior nurse-led

Every referral, assessment and care plan sits with an experienced registered nurse. No call centres, no handballing.

Clinical only

Wounds, IV therapy, devices, medications, monitoring and palliative support. If it isn’t clinical, we’ll tell you straight and point you to someone good.

Plain language

Families get the truth in words they can use. Clinicians get notes in the format they need.

Updates that arrive

Referrers hear from us without chasing. The reporting loop is the product, not an afterthought.

Richard Jones

Founder & Clinical Lead

Richard has spent close to three decades at the sharp end of clinical care, working across emergency, intensive care, retrieval, and flight operations in remote and austere environments. Throughout his career, he has led teams, mentored nurses, and cared for patients in some of the highest-pressure clinical settings imaginable.

Just as importantly, Richard has seen what happens after patients leave hospital. He has witnessed firsthand how gaps in community care lead to avoidable ambulance call-outs, emergency department presentations, hospital readmissions, and unnecessary stress for families. Those experiences shaped his belief that many crises can be prevented with timely, experienced clinical care at home.

CareLink was built from that philosophy. Every care standard, escalation pathway, and clinical decision framework reflects Richard’s commitment to delivering the right care at the right time, helping people stay safer at home while reducing pressure on the wider health system.
