The DVA pathway

How DVA nursing works.

GP or hospital referral

Your GP, treating doctor or hospital discharge planner refers you for community nursing, usually using the DVA D904 referral form.

Clinical assessment

A senior CareLink nurse assesses your clinical needs at home and develops a care plan in line with DVA requirements.

Care delivered at home

Nursing visits as set out in the assessed care plan, from wound care to medication support and chronic condition monitoring.

Review and reporting

Care is reassessed regularly, and your GP is kept informed throughout. If needs change, the plan changes.

Clinical care, not home help.

  • Complex and routine wound care

  • Post-hospital and post-surgical nursing
  • Palliative clinical support alongside your care team
  • Chronic condition monitoring and clinical review
Eligibility and cost. Eligibility for DVA community nursing depends on your DVA card and assessed clinical need. For eligible veterans, nursing is funded by DVA under the assessed care plan. We help you and your GP confirm entitlement before any care begins, so there are no surprises.
For families of veterans

You don’t have to become the nurse,
the coordinator and the advocate.

Many families come to us carrying all three jobs at once. CareLink takes the clinical load:
the wounds, the medications, the monitoring and the paperwork that goes with the DVA pathway.
